Service intervals

Can't find my hand book but I know service was carried out at 8000 when is next one due ...please ..(oil/filters every 6000??)

12,000 is the next one. Same as 4,000 but with an air filter change I think.

What actually gets changed and when? Does the oil and filter come out every 8k miles, and the air filter every 12k miles?

Interestingly as my bike is now 2 years old I've decided to start servicing it myself.
One thing I wanted to do was to have a check list so that I can tick off what I should be doing.
Is there anything like this available anywhere?
If not would anyone be willing to help out producing them? The idea being that people can then download them and print them to add to the useful bits of info that store for your bike.
