I think this has already been mentioned in passing in a couple of other threads in the past, but it looks like it's recently been updated to work with google maps now (as well as google earth) so thought I'd post it here incase anyone didn't know about it.
I've been planning a trip into Scotland, but it's ended up being delayed for a couple of days for various reasons, so while I'm sorting things out to leave (tomorrow hopefully) I tried redoing my route using this software:

I'd tried a couple of times earlier using google maps directly and one of those converters to convert it into a tomtom itinary file, but it kept screwing up in google (seems there's only so many destinations it handles before it goes screwy), and when trying to load it into tomtom it missed some of the roads I wanted to travel along.
This seems to have done a great job so far though and much easier to use. Also allows you to name all your waypoints, much easier to plan exactly the roads you want to take, and even do a test ride watching a little motorbike ride along the maps or satelite images. I haven't tried loading the itinary into tomtom yet, but looking at the file it's exported it should work fine I think (or be easy to adjust).
Well worth a look if anyone hasn't already seen it.
Right better get off and try to get organised before I lose another day.
P.S. Although it works with google earth, it's google maps you want to use it with as that's much better!
