I think most of the rubbish fuel comes about due to the standing time.
A fuel tanker is supposed to stand for 30 minutes before delivery to allow the fuel to settle, I believe the storage tanks of a station are supposed to be the same.
Some times the fact that the down time cost money means they cut the time to the bone and the fuel has not fully settled.
